NOTE: Online access will close 4:30 p.m., November 22, 2013
2. Stop by S-329 from 9:00 a.m.-2:00 p.m. on November 26, 2013 & meet with personal representative to complete enrollment form
3. Complete attached enrollment form and return to HR no later than 2:00 p.m., November 26, 2013
