SF Students COVID-19 Self-Report Form

If you are not feeling well, please do not push yourself to go to campus. Especially during the current pandemic, your professors will work with you if you need to stay home because of illness. If you 1) tested positive for COVID-19 or 2) experienced any symptoms of COVID-19 within the past 10 days, or 3) believe you have had close contact with someone who has COVID-19 within the past 14 days, please complete the COVID-19 Disclosure Form.
