As per the SF COVID-19 Exposure Protocol, SF students whom have tested positive for COVID-19 within the past 10 days, have experienced any symptoms of COVID-19 within the past 10 days, or unvaccinated students whom have had close contact with someone who is COVID-19 positive within the past 10 days, are required to complete this form and may not come to any SF location until they are cleared to do so by an SF Contact Tracer.
A Contact Tracer will call the student to conduct a phone interview as a follow-up – usually within 24 hours – so students should be prepared to discuss their health status.
